Not necessarily: Fluid can back up into mastoids related to inner ear problems with fluid and sometime infection. Infection in mastoid which can result in necrosis and bone involvement can have fluid in mastoid air cells as well. Sinus infections involve paranasal sinuses frontals (above eyes) ethmoids medial to orbits, and lateral to nose, and maxillary inferior to orbits. These are facial in location.
