See doctor : If he has had recent sinus symptoms that blood in his phlegm could just be from mucous that has drained from some irritated sinuses. If he has been coughing, blood could be coming from his lungs . Either way, please see a doctor to discern his risks. Need to rule out more serious causes of bloody sputum . That may entail a chest x ray. Too difficult to say without an exam. Good luck .

See provider: He should see his primary care provider as soon as possible as long as there are just streaks but if there is free flowing blood like with a paper cut on a finger he should go to the emergency room. More history would help localize the cause.
