Microscopy: The best way to know what this is, is to get a sample of the blister and fluid and examine this under a microscope. Also culture both and view with gram staining, if it's a bacterial problem. There are other infectious and immunologic problems that are best evaluated by a skin specialist, or dermatologist, or possibly an allergist
