Neutral position: For joint exams, it is best to see the joint in a neutral-nonstressed position. Because of the contours of the muscles in your leg, gentle support under the knee(with pillow/roll)will keep the knee more neutral and allow visualization of the tissues around the knee. A pillow under the knee would hyperextend the knee-causing stress and less visibility certain soft tissue components of the joint.
