Usually asymptomatic: Lipomas of spermatic cord are quite common, especially in males with a high BMI. Not symptomatic unless there is an inguinal herenia in addition & symptoms would be from hernia rather than lipoma

Not likely but may : some large lipomas of the cord may cause pain by being close to nerve endings in the inguinal canal but most of the times they are incidental and present along a subtle symptomatic hernia

Hard to know: Lipomas on the spermatic cord are usually incidental findings at the time of surgery. Whether they cause pain is hard to determine.

Not generally: cord lipomas are very common and should not cause discomfort unless very large. Cord lipomas are located in the inguinal canal where hernias occur. Pain in this region is more likely due to an inguinal hernia
