Rash or infection?: If it is very tender it could be associated with an infection. If it tends to be more itchy it could be a rash or an insect bite. Observe his behavior and if he gets very tender, more swollen and redness spreads take him to get seen by a medical provider. If it stays the same and it is itchy you may try anti itch cream that you may find over the counter.

Insect bite: It is possible it is due to an insect bite, it would be wise to check the entire body for similar lesions. consider possible " bed bug bite" You my use hydrocortisone, but visit your pediatrician.
