A little big: Around ovulation, the follicle measures around 2 cm, so this is a little bigger. It's not so big, though, that it can't be watched. It will likely resolve on its own.

Probably not: Each month when a woman ovulates, a cyst forms at least 2 cm in diameter. After ovulation that cyst may increase to 6 or 8 cm and still be normal. It will usually go away over the next few weeks.

Depends: At 4 1/2 cm if it's a simple cyst, then it may not need to be operated on at all. At that size, many of these cysts do resolve on their own, considering, of course, that they are simple and filled with solely fluid. Having said that, if the cyst is causing twisting of the ovary, if there's severe pain, then doctor might have other recommendations.
