Depends: where is the swelling and reddness? if anterior to ear it may be an infected branchial cleft remnant or a preauricular sinus. see your pediatrician.

However, trauma to the outer ear is a medical emergency due to excessive bleeding causing damage to the cartilage. This can occur from falls in children. Your son should see his doctor now to ensure correct diagnosis and treatment.
