Different strokes: ACL injury can present from mild (partial tear) to severe (complete tear) usually caused when the knee joint is forced to hyperextend & can happen through various mechanism of injury; therefore, ACLs tears do not look the same each time it happens

Probably not: The complete rupture would be identifiable on the current scan, whatever sprain/partial tear that was present 2 years ago probably wouldn't be identifiable.

Yes, with the recent: injury there will be evidence of bleeding and swelling in that area while the previously injured part would have some scar tissue associated as well. In surgery the difference can be made easily as to which part of the Acl was injured previously & recently.
