Eczema?: It's possible you're dealing with eczema, which causes one to have extremely dry patches of skin that can become irritated and itchy and are often resistant to most over the counter lotions and creams. Have you tried Vaseline? You may need prescription medications like DermaSmoothe or courses of a topical corticosteroid in cream or ointment form. Talk to your doc.
