Labs in biliary dz: Labs offer only one window into the disease process that's going on. So your question can't really be answered. The liver profile (LFTs) contains Alkaline Phosphatase which is a surface enzyme of the cells lining the biliary tree- that should be elevated. GGT is another surface enzyme that should be elevated, but it's not a routine part of the LFTs. Imaging tests and other clinical info is needed.

Need more info: generally elevated direct and total bilirubin is a sign of common bile duct obstruction, which is sometimes associated with cholangitis (fever, elevated white blood count, and pain) but radiologic tests are needed to make a diagnosis
