Kidney failure: Patients with diabetes and high blood pressure are at risk for kidney failure. The increasing creatinine from 1.6 to 4.9 in 3 months is very worrisome for acute kidney failure superimposed on chronic renal failure. If he has symptoms of nausea, vomiting, decreased alertness, poor memory, he should be evaluated immediately in the ER or by his kidney specialist.

Acute renal failure: Your father's chronic kidney disease (CKD) has worsened dramatically over the past 3 months. While is consistent with acute renal failure superimposed on CKD. Causes could be obstruction of his urinary tract or nephrotoxic agents, among others. Have him see a urologist to rule out an obstruction and if he has uremic symptoms, vomiting, itching, anorexia, have him go to an ER to evaluate him.
