Pyelonephritis: If you are sure that this is an upper UTI (involving structures above the bladder) you need to be seen ASAP by a doctor. Do not know much about ERs in Ireland, but if available would go there. Or perhaps an urgent care facility. You need to be evaluated, cultured and treated. This is a potentially very serious condition. Do not ignore it. Best wishes.
