Need diagnosis: If you push in front of the ear, you may have a problem with the jaw joint. Another possibility is the use of a Q-tip may be irritating the skin of the external ear canal causing external otitis, which is an infection of the ear canal skin. Stop using instruments to clean your ear canals. I presume the term "no-hole ear" means you have no perforation of the ear drum.
