Hypothyroid?: T4 is the main thyroid hormone made by your thyroid. If it is low and you are hypothyroid, then the pituitary hormone TSH, which controls the thyroid, should be elevated as it attempts to stimulate the thyroid. But it is not clear what "extremely low" means. There are normal physiologic reasons why T4 can be low. Best to talk to your doctor.

Hypothyroidism: Low T4 is an indication of the thyroid gland not producing enough thyroxine hormone which is necessary for overall body metabolism. Need further work up as to why your T4 is low. Will ultimately need thyroxine replacement therapy. Symptoms of fatigue , constipation, dry skin, weight gain and hair loss will continue without thyroxine replacement.
