Could be both: PCOS is a hormonal disorder and affects 15% of women. Irregular bleeding , ultrasound criteria and elevated male hormones are common. Many bleeding patterns can occur in PCOS. Menorrhagia refers to heavy periods, but could be from any cause. So likely PCOS is the underlying issue
