It depends: "Fatty liver" has many forms. If there is just fat (steatosis), this is easily reversible. However, if inflammation (hepatitis) has set in, it may be less reversible. This form of fatty liver, called steatohepatitis, can lead to cirrhosis. In general, weight loss and exercise are the key treatments for fatty liver diease, as well as control of diabetes or hi cholesterol if present.
