Can vary a lot: If the sore lymph nodes are just in the neck, haven't been there long, and are associated with a normal sore throat or cold, your brother may not need to do anything other than wait for it to go away. If he has swollen and sore lymph nodes in multiple places (arm pits, groin) and especially with a fever, he should see a doctor - and not necessarily an ENT. Cost varies by location. Call first.

Lymph adenopathy: Need to see primary care first for work up and full exam. Maybe infection or inflammation. Seeing primary maybe more cost effective; less than 200.00. May resolve problem without specialist which my double that cost. Especially in a young man .
