No lab-type test: Mental illness diagnoses are almost entirely based on self-report (or reports of family) about behaviors, feelings and patterns of them over time. Further confidence is sometimes taken from results of using medications. If it reduces symptoms, you are believed to have the disorder. There are projective tests (Ink Blots, etc.) and questionnaires where answers are like others who have the disorder.
