Depends: At 4 1/2 cm if it's a simple cyst, then it may not need to be operated on at all. At that size, many of these cysts do resolve on their own, considering, of course, that they are simple and filled with solely fluid. Having said that, if the cyst is causing twisting of the ovary, if there's severe pain, then doctor might have other recommendations.
