Back pain: It's unlikely that her back pain is due to diabetes or to heart disease (which she apparently doesn't have anyway). The most likely cause is strain of the muscles, intervertebral disk disease, or arthritis of the facet joints of her spine. In most cases in her age group (45?), this will get better by itself with time, rest, heat, massage, and analgesics.
