Orbital problem: I would see anophthalmic orbital specialist.Ou probably need an MRI of the head and orbits. There could be something pressing behind the eye causing eye lid to puff up and eye to turn in.It could be an inflammatory process or tumor.Get it checked soon.

Eye muscle: Deviation or turning in of the eye means muscle weakness or nerve damage and the problem could be in the eye itself or the brain. Since the eye is puffy, i recommend that he see an ophthalmologist as soon as possible for diagnosis and treatment.
