High ankle sprain: The high ankle sprain involves the anterior/inferior tibial/fibular ligament and may involve the interosseous membrane between the tibia and fibula. Caused by the wedge-like shape of the talus in the ankle mortise with the rotational moment of the injury, you can stretch, sprain or tear these structures, leaving the ankle unstable during gait. The only definitive evaluation is an MRI.
