I know about this: Yes, they need a LOT of FDA approval including 1 probably 2 double blind trials. There is the problem of blinding and it can be tricky to have a true blind.

It means that both the patient and the doctor do not know which one is the real medicine and such they can appreciate better its effect. For vaccines we can measure the antibodies triggered .A very high quantity (titer) is protective. So the procedure is very objective. The problem it is not the efficiency but the complications
