my grandma is 89 years old her blood pressure goes up all the time but she keep having hallucinations and when i talk to her she answers differently?

She needs eval asap: If this behavior is new, she needs an emergent appt/emergency room visit as she may be having a stroke. If is her baseline, then she still needs an apt in the near future to get her BP under control and assessed for confusion and possible dementia causes, etc.
