Depends: If there is severe infection present, often the dentist/oral surgeon will not extract the tooth right at the first appointment due to the possibility of spreading the injection upon injecting novocaine. But being put to sleep or nitrous oxide or sedation dentistry may help. Modern dental techniques are vastly superior to the old days in terms of pain management.
