May Be Trigger point: Your painful knots in upper back are likely located in your muscle and more likely are what is called trigger points. If you push hard on them, they should elicit pain that often travels up and down the muscle. Trigger points can be treated conservatively with heat and massage and also ibuprofen. Sometimes an injection of cortisone by a pain physician in the knots can help relieve them.
