May Be Trigger point: Your painful knots in upper back are likely located in your muscle and more likely are what is called trigger points. If you push hard on them, they should elicit pain that often travels up and down the muscle. Trigger points can be treated conservatively with heat and massage and also ibuprofen. Sometimes an injection of cortisone by a pain physician in the knots can help relieve them.

I do not know if: Your knots or trigger points are related to another medical problem. Acupuncture can be superior for breaking down trigger points as can gua sha (scraping). Take care.

Treat disease: Your best option is to treat the Fibromyalgia. The 3 meds used for symptom control are lyrics, cymbals and savella (milnacipran). Avoid pain meds they can make this disease worse over time. The primary treatment for fibromyalgia is following good sleep habits, medications like Elavil at bedtime and aerobic exercise. The Tx is not easy but it works.
