Needs evaluation: Conditions such as uveitis which are responding to treatment like methotrexate, at least stay at the same level of function and hopefully are improving. Yours sounds like it is worsening and may need additional or different treatment. You should be evaluated as soon as possible by your treating ophthalmologist (not optometrist i hope).

Insufficient therapy: You are on a relatively low dose of Methotrexate and you are taking the less effective form--oral. Most adults tolerate 20mg of weekly Methotrexate very well. Injected liquid Methotrexate under the skin is painless, more effective (completely absorbed), and fewer side effects. You may also need a tnf-inhibitor like Humira or remicade (infliximab). See a rheumatologist also to check for sarcoidosis and jia/jra.
