Could be: 4 mm is relatively small. Malignancy cannot be diagnosed by an x-ray, ct scan or mri. The diagnosis of cancer requires a piece of the tissue to be examined with a microscope. There are characteristics on imaging though that make malignancy or less likely. Hope this helps.
