I have had an implant for about 6-7 years ago. things were good, now there is a fistula. should i be concerned? periodontist said to just monitor.

Cause ?: What was determined to be the cause of the fistula? It usually means a draining abscess. Abscess can result in inflammation, which can lead to bone loss. Your periodontist knows your situation best and it would be hard to second guess them over the internet, sight unseen. Monitor it closely.

Fistula=infection: A fistula is a clear sign of infection that is draining. If this is present near the implant it should be checked to determine whether or not it is failing or if it is coming from another tooth. Sometimes fistulas will drain a tooth or two away from the culprit. Your periodontist should be able to determine if the implant is suspect. If it is fine then have your dentist look at it.
