Pyleonephritis: Outpatient treatment of a kidney infection is reasonable under many situations, but if you are still running a fever you need to be rechecked. The culture report needs to be checked to be sure the antibiotic you are on is correct. If you are, but are not responding, then you may need to be hospitalised for IV antibiotics.

Correct antibiotic.: 1st need to check that bacteria causing your kidney infection are sensitive to antibiotic taken. Change to an appropriate antibiotic if necessary. Macrodantin (nitrofurantoin) does not work for kidney infections as it does not penetrate tissue. Unfortunately most bacteria causing urinary tract ; kidney infections are resistant to amoxicillin. Hope you had a urine culture done, if not please do it now.
