Your doctor can help: It depends on finding the cause which varies in different folks. I am sure your doctor has some advice for you as to how to manage it further. Surgery is required if the fluid build up is progressively increasing and if you have symptoms of which is called, constrictive-:pericarditis or tamponad. Only your cardiologist can assess this and guide you further.

It depends: Pericardiocentesis is indicated in pericardial effusions more than 2cm in echocardiography. This is a rough guideline but if you have any other medical problems and symptoms, cardiologist or cardiotheoracic surgeon will make a decision.

Pericarial effusion: A pericardial effusion greater than 1.0 cm in thickness is significant. As to the trreatment and the significance of the echodense material, you need to call your cardiologist for an interpretation and an assessment of its clinical significance.
