Needs evaluation: The most common cause of vision change is a need for improved eyeglasses. But there are many other causes, some treatable, and some not treatable if neglected. If you are having a dramatic decrease, you should see an ophthalmologist as soon as possible.

Changing refraction: There are a number of reasons why your vision prescription may be changing. At your age, it should be stable. You should see your eye doctor for a complete exam. At a minimum, your doctor can document the rate of change and offer treatment if appropriate.
