Can you clarify?: Tobacco is a factor that contributes to a tumorous growth of langerhans cells in the lung, a variant of "histiocytoxis x" called "eosinophilic granuloma". It is not usually so serious as the COPD itself. I am glad he has you to help care for him. Best wishes.

Depends on number.: This condition if all related to COPD is usually increase in hemoglobin as well. Monitor WBC and may need to see a hematologist. When WBC reach a certain number >100k then that's a problem. Need to know the number and differential of the cells. Meaning which specific WBC line is abnormal.
