Central stenosis: The MRI is a picture. It is one piece of the puzzle. The other important pieces are your symptoms, and the examination. "flattening" of the cord is much more significant if you are having symptoms like numbness, weakness, or bladder and bowel changes. You should definitely be counseled by the provider who ordered the study.

Depends on: Symptoms. Imaging findings by themselves are not enough for guidance. Your physical exam findings have to be correlated with the MRI to determine if what is seen on imaging is important to you.

Neurosurgeon: With your findings of stenosis, severe arthropathy, and broad disc protrusion i suspect you are having pain. See a neurosurgeon for an evaluation and treatment to start.

Spinal stenosis: From what you are describing on the mr i would highly recommended seeing a spine neurosurgeon for further evaluation, the central canal stenosis can be quite worrisome and based on your age of 42 you will likely need to have something done or at the least evaluated.
