Fist size?: A bump nearly the size of your fist sounds like a large bump to me. Even though, it may be related to your injury-however other things unrelated to the injury may be the cause as well. Without examining you-it will be hard to say. Go to see a doctor and get it examined.

Difficult 2 treat: Without a dr. You need to get a doctor involved, as the size of a fist is huge, and warrants getting further work up, and imaging if need be. Also, a 2nd opinion from an osteopathic doctor would be helpful to determine if it can be treated osteopathically to not need surgery or not.You can't possibly sit or drive comfortably, and need donut pillow to sit on, and decompress your sacrum, and injury.
