"threaded splinter": It sounds like you may be referring to screw hardware removal from an ankle syndesmosis injury for which two screws were placed across your fibula into the tibia. Theses screws are commonly removed (akin to a "threaded splinter") as an outpatient procedure once adequate healing has occurred. Pain should be considerably less than the perioperative pain who had at the initial surgery.
