Possible causes: When you strain, you increase intra-abdominal and intra-thoracic pressure which favor acid reflux as the cause, but pain with cough and change in position suggests pericarditis or possibly pleurisy. A chest x-ray, echocardiogram, physical exam and sed rate would rule out pericarditis and pleurisy.
