Spots on fingers: Unfortunately, it's hard to make a diagnosis with the limited info provided. Septic arthritis would most likely involve larger joints and would present with red, hot, swollen joints. Your presentation doesn't sound like it unless there is more to the story. I would make it to your doc to show the rash to him/her to get the right diagnosis.

Need to be examined: Your symptoms are difficult to assess without a proper examination. Go see the doctor. A skin biopsy may be recommended.
