Piriformis syndrome: The sciatic nerve runs through the piriformis muscle which is deep in the buttock region, and this nerve can get entrapped and inflamed as a result. This would result in moderately severe back pain and could result in symptoms of the calf and the foot, as the sciatic nerve innervates this part of the lower extremity.
