I have a staghorn stone the dr said a pnl is necessary.i'm passing large fragments of the stone. should i get a second opinion or ask for a new ct.

Body habitus: Multiple factors should be considered when treating "staghorn" calculi: stone composition, body habitus (size/shape), co-morbidities, previous surgical approaches, surgeon experience, renal anatomy, ....... Best combination approach for most is probably major debunking by pcnl followed by eswl.
