Hard to say: Without a musculoskeletal exam or even a picture, it's hard to say. The eyelids sound like blepharospasm. The shoulder issue may be a version of dystonia. If it's originating from the neck it could be a cervical dystonia. Botox could help.

Cannot say .....: Really need more information. Need to know more about your neurological examination. Could all be unrelated or be a form of segmental dystonia. Need to check with your doctor and perhaps another opinion from a different neurologist.
