Very difficult: Permanent voice loss completely is usually secondary to a paralysis of the nerve innervating the vocalis muscle. ( the muscle of the vocal fold). If you are constantly abusing your voice as in shouting, screeching, or loud signing, you will develop hoarseness from nodules, polyps or granulomas, but you should never develop aphonia (complete loss of the voice).
