Prob not piles: Hemorrhoids are usually assoc with painless rectal bleeding. The symptoms you describe are consistent with an anal fissure (tear); usually treated with stool softeners and topical Nitroglycerin or cardizem. They sometimes require surgery. He must see a GI or surgeon because cancer must be ruled out!
