Have it checked out: Severe pain on the top of the foot feeling like a charlie horse is not a common finding. Your recent activity history, how much your run or walk for exercise can affect this. If it's acute and relieved by pain meds, then that's not as much a problem as if it's been continuous without relief. Best is to have it checked out by your doctor to see what the exam shows, and to know for sure what it is.
