See plastic surgeon: Pigmented lesions need to be reviewed by a plastic surgeon or a dermatologist. If necessary these can be biopsied, usually, exchanging a worrisome mole with a small scar.

Yes: You should have a baseline dermatological exam to evaluate the mole. This is particularly important since you have discomfort and are not able to look at it yourself. If the lesion looks suspicious your doctor may remove it.
