Chronic obstruction: The calyces are the beginning of the collecting system within the kidney. They are sharp when not blocked. With obstruction they dilate. Blown out calyces indicate a very dilated system which represents a very obstructed system which is chronic in nature. It is important to ask whether our not that kidney has any cortical tissue remaining. The cortical tissue is the functional part of the kidney.
