Many possibilities : something like this needs to be worked up by a doctor who will run a series of blood tests to determine the cause

Define "hunger pang": Sensation of hunger is related to contractions of stomach muscles, & lead to hunger pangs once they become severe. Are triggered by hormone Ghrelin (released if blood sugar levels drop with prolonged periods without eating). Hormones Peptide YY & Leptin cause the reverse--the sensation of being full. Irregular meals make hunger pangs worse; older adults may have less violent stomach contractions.
