If a dr. has a fellow in surgery, is the fellow allowed to do the procedure instead of the dr. i paid for? wouldn't they have to ask my permission?

Ask: Ask the surgeon the facility's policy. You are not paying for a specific surgeon, you are paying the facility to have a good and successful surgical procedure. You have to ask, but generally in a training or teaching facility, the surgery fellow may do part or all of an operation under the direction of the attending (chief) surgeon.
