Needs evaluation: A hard mass in or near the thyroid is worrisome, especially if the fine needle aspirate may perhaps not have yielded the abundant cells required for diagnosis. Two entities to consider are a heavily calcified papillary thyroid carcinoma and a riedel's struma.

Ultrasound yes...: If adequate samples were obtained by FNA, and the cytologist is satisfied with the quality of the fine needle aspirate, then I would take the result at its face value. But, make sure to have that ultrasound study. Ideally, FNA is best when performed under ultrasound guidance.
